Endometriosis in TCM

Chinese medicine does not have an exact equivalent to endometriosis, but the symptoms map closely to the pattern of Blood stasis — fixed, stabbing pelvic pain, clotting, dark menstrual blood, and the cyclical worsening of symptoms around menstruation.

Additional patterns often present alongside Blood stasis: cold-damp accumulation, Liver Qi stagnation, and Kidney deficiency — each adding their own symptom layer and requiring specific treatment modifications.

Pain Management Without Over-Reliance on NSAIDs

Many women with endometriosis rely on high-dose NSAIDs (ibuprofen, naproxen) for pain management. While these are appropriate short-term, long-term use causes gut lining damage and rebound effects.

Acupuncture and specific herbal formulas (such as modified Shao Fu Zhu Yu Tang — Minor Decoction to Expel Stasis from the Lower Abdomen) can significantly reduce pain and dependence on analgesia over a course of treatment.

Fertility and Endometriosis

Endometriosis is associated with up to 50% of infertility cases. TCM treatment aims to improve the uterine environment, reduce the inflammatory burden on developing follicles, and support implantation — working alongside gynaecological management or IVF as appropriate.

A 2021 systematic review found acupuncture significantly reduced dysmenorrhoea pain scores in endometriosis patients compared to sham or pharmacological controls, with improvements in CA-125 (an endometriosis inflammatory marker) in several trials.
